Let's Get Ready for Your Pet’s Surgery!
Please fill out the following form prior to your pet’s anesthetic procedure. When you arrive at 7:30am the morning of your appointment please keep your phone available and remain in or around your car until further communication with us. When you arrive for drop off please call us at (303) 680-7305.
Reminder that pets undergoing anesthesia or sedation can NOT eat breakfast the morning of the appointment (except guinea pigs, rabbits, and chinchillas)! Water is allowed but all food should be withheld after 10pm the night before your pet’s appointment. *There will be rare exceptions - please follow instructions specific to your pet if you were told otherwise*
A staff member will call to answer any questions you may have about the consent form and then come out to retrieve your pet shortly after arrival. A receptionist will call to take a credit card number over the phone for the surgery deposit of $350, which is applied to the balance for surgery.

Surgeries are performed in the first half of the day and then patients need several hours to recover before going home. Most pets can go home after 3pm but the doctor will call after the procedure to give you an update on how things went and schedule the discharge appointment time.
If this is your pet’s first visit with us please fill out the New Client/Pet Form.
